I was taught by professors and residency mentors to avoid the use of corticosteroids in patients with infections. My patients were horses, and misuse of corticosteroids is a real problem in horses. One summer at the referral hospital where I worked we had several cases of severe viral pneumonia in older foals. They kept getting worse despite intensive treatments, and I remember thinking, “I need to bite the bullet and give these guys steroids.” I did and the improvement was rapid and remarkable. From then on I regularly used corticosteroids in this type of case, and I taught my residents to do the same. So one if the first things on my COVID preparation list was to get some steroids. Glad this is now an accepted treatment in people. No one should die without the benefit of steroids!
